What is Covid-19 Informed Consent?
Informed consent is a person’s voluntary decision about healthcare that is made with knowledge and understanding of the benefits and risks involved.
The Covid-19 Informed Consent template is a document signed by your client prior to receiving treatment during a covid-19 outbreak. The client acknowledges that they have been informed of the potential risks of receiving or continuing treatment during a covid-19 outbreak, and wish to obtain treatment despite the risks.

Do I need it?
While there are no legal requirements to have a covid-19 specific informed consent document, practitioners treating clients during the covid-19 outbreak, may wish for their client to sign one to protect both the practitioner and the client during the outbreak.
What does it cover?

- Client confirms they are not experiencing any covid like symptoms

How do I use it?
This document should be signed by the client before the client receives treatment or before they choose to continue treatment, during covid-19 outbreak. For example, for initial consults this document can be provided along with your initial paperwork or questionnaire or filled out in by the client in clinic (if applicable) before their consult. If a client is choosing to continue treatment, have them sign it before their next consult.
NOTE: Before treating clients during an outbreak, a practitioner should inform themselves of any laws, or public health orders in place at the time of the outbreak to ensure they are legally allowed to continue to treat clients, and to ensure they follow other legal guidelines and restrictions that may be in place when providing treatment (i.e. mask wearing etc). If you are unsure, please consult your association and/or a lawyer.
Does this replace my usual informed consent document?
No as it only deals with the risks associated with your client receiving treatment during a covid-19 outbreak. Ideally you would have your standard informed consent document, and then only use this one in addition to that during a covid-19 outbreak where risk may be heightened. If you feel confident doing so, you could combine the clauses from this informed consent, into your usual informed consent document however our preference and recommendation would be to keep them separate, as you may only need this one during specified periods of time (during an outbreak).
